PCTs are responsible for ensuring that practices receive an indicative budget that reflects the needs of their population as accurately as possible. This allows a practice to access a 'fair share' of the resources available to the whole of the PCT for its patients. The Department has updated the toolkit that can be used to determine weighted capitation indicative budgets at practice level. The Independent Mental Health Advocate (IMHA) service commissioning guide has been prepared for the introduction of IMHA services on 1 April 2009. The guide outlines the statutory IMHA role, discusses good practice for IMHA services and outlines the commissioning process required to ensure high quality IMHA services are provided for qualifying patients under the Mental Health Act 1983.

The Improvement Foundation’s Healthy Communities Collaborative (HCC) model combines community development and quality improvement, lay knowledge and professional support in an innovative community-led approach which is specifically aimed at reducing health inequalities. This paper describes the key characteristics of the model and uses an outcomes framework to discuss indicative results from an HCC initiative to promote the earlier identification of people at risk of cardiovascular disease implemented in nine spearhead local authorities.

Practice-based commissioning (PBC) is one of the cornerstones of health policy reform, but has it delivered its intended benefits? Its objectives were to encourage clinical engagement in service redesign and development, improve services for patients and make better use of resources. Through in-depth interviews with GPs, hospital and primary care trust staff in four sites, this Kings Fund report assesses the progress of PBC and identifies the barriers that are limiting success. Using the evidence from the four case studies and from previous research, it suggests ways forward for policy-makers.
